Remote Patient Monitoring & Chronic Care Management

About the Role

Truentity Health is hiring a Medical Assistant to support our chronic disease management program. This is a full-time, site-based role where you are the primary driver of patient recruitment, enrollment, and ongoing care management — supported day-to-day by a clinical pharmacist and supervised by our physician team.

This is a high-impact, patient-facing position for an MA who thrives working independently, is energized by meaningful patient interaction, and wants to be part of a next-generation model of chronic disease care.

Key Responsibilities

Patient Recruitment, Enrollment & Onboarding

  • Serve as the primary point of contact for recruiting eligible patients into the RPM and CCM programs.
  • Conduct initial patient visits on-site, including eligibility verification, insurance confirmation, and program enrollment.
  • Complete patient intake, set up cellular-connected monitoring devices (blood pressure and blood glucose monitors), and educate patients on proper device use and program benefits.

Daily Monitoring & Care Management

  • Review patient-submitted vital signs daily via the RPM monitoring portal.
  • Identify clinically abnormal readings and escalate concerns to the supervising pharmacist or physician team per established clinical protocols.
  • Conduct routine patient outreach, maintain and update care records, troubleshoot device issues, and ensure monthly monitoring compliance.
  • Schedule and manage follow-up contacts to keep patients actively engaged in the program.

Program Operations & Documentation

  • Execute standardized program workflows, task lists, and training protocols to deliver consistent, high-quality care.
  • Maintain accurate records and support quality assurance and compliance efforts.
  • Assist with data collection related to patient outcomes and program performance reporting.

Qualifications

Required

  • Experience in outpatient care, chronic disease management, or care coordination.
  • Strong patient communication and engagement skills — comfortable conducting frequent one-on-one patient interactions and outreach.
  • Comfortable using telemedicine platforms and clinical monitoring or EHR software.
  • Highly organized; able to manage multiple priorities and work independently with minimal supervision.
  • Meticulous with medical documentation practices.

Preferred

  • Experience with Remote Patient Monitoring or population health management programs.
  • Familiarity working across virtual or distributed care teams.
  • Certified Medical Assistant (CMA) with active certification.
