The Role
You are the primary day-to-day partner for independent pharmacy owners and their staff as they implement and run the Pharmacy Activated Chronic Care through Trust (PACT) program through the Truentity platform. You conduct training, drive patient enrollment momentum, troubleshoot platform issues, and make sure every pharmacy in your book is hitting its KPIs across active patient engagement, clinical quality, and productivity. This is not a role with a fully built playbook — you are expected to identify what is working, document what is not, and help build the systems that make the next PSM faster to ramp. Key Responsibilities
- Onboarding and training: conduct virtual and onsite onboarding for new pharmacy accounts, platform setup, clinical workflow integration and staff training.
- Daily account management: serve as the primary contact for your pharmacy book; monitor engagement and KPIs weekly; proactively identify and address accounts at risk before churn occurs
- Patient enrollment: coach pharmacy staff on patient recruitment and enrollment workflows; help identify eligible patients from the existing Rx customer base
- Program operations: execute standardized workflows, task lists, and training protocols; maintain compliance with Truentity clinical protocols, documentation standards, and applicable regulatory requirements
- Cross-functional voice: translate real account feedback into product and operations inputs; collaborate with the Head of Customer Success on playbook development and team process improvement
What We Are Looking For
Required
- 3+ years of experience in customer success, pharmacy, or a directly related role
- Strong understanding of pharmacy operations and the day-to-day pressures of independent pharmacy owners and staff
- Track record of building and sustaining customer relationships in a high-touch, account management model
- Comfortable with healthcare technology platforms.
- Highly organized; able to manage multiple accounts simultaneously with minimal supervision
- Meticulous with clinical documentation and customer account records
- Strongly Preferred
- Prior experience as a Certified Pharmacy Technician (CPhT) — candidates who have worked inside an independent pharmacy workflow have a meaningful advantage in this role
- Familiarity with pharmacy management software: PioneerRx, Rx30, or BestRx
- Experience supporting RPM, CCM, TCM, or population health management programs
- Experience working across virtual or distributed care teams
